To make it easier for patients to find their prescription number and store information on your prescription label, we include a sample Rx label on the registration screen. This reduces support volume and increases completed registrations.

To get the highest quality image, we prefer that you send a high-resolution image of a sample prescription label. If you can't send us an image, you can mail us a printed copy, to:

If neither of the above are available, please take a picture of a sample label using the highest resolution possible and send it over.
